I agree to terms & conditions provided by Soccer Mavericks. I acknowledge that in checking this form, I waive Soccer Mavericks of all liability related to my child's health. I acknowledge that I will be charged a prorated amount for the remainder of the month (from the first week of my child training) then charged on the 1st of every month. By completing this registration, I understand and agree to the following terms regarding enrollment in Soccer Mavericks programs:

30-Day Cancellation Notice: I acknowledge that all cancellations must be submitted in writing (email or official cancellation form) with a minimum of 30 days’ notice prior to the next scheduled billing date. Final Payment Responsibility: If I submit my cancellation notice less than 30 days before the next scheduled payment, I understand that I am responsible for the final payment due during the 30-day notice period. This final payment will be processed as scheduled, and no refunds will be issued for unused training time within that notice period.

Billing Obligation: My enrollment and payment obligation remain active until the 30-day notice period has been completed and the final payment has been made.

Non-Attendance: Failure to attend training sessions does not constitute cancellation. I understand that I remain financially responsible during the notice period, regardless of attendance.

Season Payment Refunds: If I choose to pay the program in full for the season, I understand that no refunds will be issued after enrollment. Any refunds requested will be at the sole discretion of Soccer Mavericks. Refunds, if granted, may be subject to prorated deductions and administrative fees.

Confirmation: All cancellation requests will receive written or email confirmation from Soccer Mavericks once processed.
